Transformative program recognized for identifying patients in need of clinical support and changing the trajectory of health outcomes

This prestigious awards program honors the visionaries, products, and organizations that are revolutionizing the world with bold innovation.
Measuring Healthy Days with Included Health is based on the belief that ongoing engagement with people in proactive, personalized ways leads to improved health outcomes for all. Embedded as a core component of Included Health’s all-in-one, personalized healthcare offering, Healthy Days is an outreach program that uses a patient-centered measure originally developed by the U.S. Centers for Disease Control and Prevention to understand health-related quality of life, inclusive of both physical and mental well-being. The intent is to better manage population health by offering simple, proactive, and personalized engagement.
“Asking members to regularly consider how often their physical or mental health is ‘not good’ can have a big impact on getting people healthier by addressing their unmet medical needs,” said Ami Parekh, M.D., J.D., chief health officer at Included Health. “By prompting our members every 90 days to reflect on their overall health, we’re proud to be helping them when they’re not at their best and, most importantly, to realize two additional Healthy Days every month.”
In 2022, more than 23,000 individuals across all 50 states participated in a Healthy Days pilot program, led by Included Health. One third of the pilot population reported 15 or more unhealthy days — more than half the days in a month. These individuals were then offered the opportunity to connect with an Included Health clinician. In 40% of these opportunities, individuals accepted the clinical support, which resulted in recommendations of virtual therapy and psychiatry visits (53% of the time), education on relevant benefits (53% of the time), connection to high-quality primary care providers (41% of the time), or referrals to case management or coordination services for longitudinal care (33% of the time). This process ultimately led the population to two additional Healthy Days per month, on average — or two additional days each month to engage in the personal, professional, or self-care activities they enjoy most.
