Included Health is a health benefit made available to you through Walmart. We are a primary guide and resource for all your medical and health care-related needs. Our services include finding you the top providers and specialists in your insurance network, booking appointments on your behalf, helping you understand expected medical costs and current billing, getting you remote second opinions, and answering any questions you may have about your health plan, medical conditions, and more.

Use Included Health when:

  • You need a primary care provider or specialist for an in-person visit. We’ll help you find trusted and experienced providers within your insurance network and can even set the appointment for you.
  • You’d like quick answers to medical questions. We can provide you with over-the-phone guidance and support from an Included Health provider.
  • We’ll help you understand all of the details of your health plan.
  • You have questions about billing. We’ll help you understand medical costs and whether the price you’re being quoted is fair.
  • You’re unsure about a diagnosis, or you’ve been recommended surgery as a form of treatment. We can arrange for you to get a remote second opinion.
  • You’re not sure which of your benefits you should use. We’ll help you explore the different health benefits available to you and when to use them.
  • You need to book a virtual visit with a provider. We’re available 24/7 for urgent care virtual visits.
  • You want to connect virtually with a behavioral health specialist. Visit with one of our high-quality providers over video.
  • You or a covered dependent would like to find trusted, LGBTQ+ affirming providers near you.

Included Health is available at no cost to Walmart associates and dependents covered by a Premier PPO Plan, Contribution HRA Plan, Saver HSA Plan, and Personalized Wellbeing Copay Plan. Walmart provides this service so that you have access to the best medical care possible.

Plan guidelines will apply for virtual care visits and any in-person visits or treatments as a result of the Expert Second Opinion or Office Visit — Included Health will find the provider, collect all relevant medical records, and (if necessary) schedule the appointment at no cost. For further details, call the Included Health Care Team at 1-800-941-1384, available Monday through Friday from 7:00am – 8:00pm CT.

All Included Health services can be easily accessed by calling 1-800-941-1384 or by downloading the Included Health mobile app. For certain Included Health services, a member from your Care Team will reach out to you to learn about your exact needs. They’ll take care of everything, from finding you the right provider to collecting your medical records. Expect a care team member to follow up with you after you’ve seen the recommended provider, to answer any questions you may have and start additional cases if needed.

Included Health providers span all conditions and specialty areas, including: back and knee pain, cancer, heart disease, arthritis, behavioral health disorders, migraines, digestive system disorders, dermatology, pediatrics, ACL tears, and much more.

Typically, you’ll only pay your regular copay and office visit fees for seeing an in-network provider. For further details, reach out to the Included Health Care Team.

Included Health selects providers based on their education and training, patient outcomes, and location. As a result, we’re able to match you with highly experienced providers and specialists who are best suited to your personal preferences and medical needs.

Most providers are glad to get the opinions of their peers when assessing complicated medical issues. Their primary goal is to get you feeling better. A good way to frame a conversation with your treating provider regarding an Included Health expert opinion is:

  • “I requested a consultation about [condition] and here’s what I learned…”
  • “Here’s what I was surprised to find out…”
  • “Dr. Smith recommended these options for my care because of these reasons…”

Any personal health information you share with Included Health is held confidentially and will not be shared by us, unless authorized by you, or where legally permissible and/or required under applicable law.
