Included Health is a health benefit made available to you through Bread Financial. We are a guide and resource for your medical and health care-related needs. Our services include finding you the top doctors and specialists in your insurance network, booking doctor’s appointments on your behalf, getting you remote second opinions from world-leading experts, answering any questions you may have about medical conditions, and more. New for Bread Financial members, cancer support. The Cancer Support Program from Included Health provides the expert guidance and care you need.

Use Included Health when:

  • You need a primary care physician or specialist for an in-person visit. We’ll help you find trusted and experienced doctors within your insurance network and can even set the appointment for you.
  • You’re unsure about a diagnosis, or you’ve been recommended surgery as a form of treatment. We can arrange for you to get a remote second opinion from a leading expert specializing in your area of need.
  • You’d like quick answers to medical questions. We can provide you with over-the-phone guidance and support from a Included Health doctor.

Included Health is available at no cost to you and your dependents who are on your Bread Financial medical plan. Bread Financial provides this service so that you have access to the best medical care possible.

Included Health is fully covered by Bread Financial and available at no cost to you and your enrolled dependents. Simply enter your information and Included Health will waive all fees. Plan guidelines will apply for any treatments as a result of the Expert Opinion or Office Visit – Included Health will find the doctor, collect all relevant medical records, and (if necessary) schedule the appointment at no cost.

Bread Financial will pay 85% of your in-network claims (5% more than normal) for certain procedures if you complete a Included Health Expert Opinion prior to surgery. The specific procedures include:

  • Back procedures
  • Hip procedures
  • Knee procedures

The procedure must be completed at an Aetna IOQ in order to qualify for the incentive.

Included Health has found that certain surgeries and procedures have a higher rate of misdiagnosis and improper treatment. Because of this, Bread Financial is requiring that you work with Included Health before proceeding with certain non-emergent bariatric procedures so that one of the country’s experts can confirm you are on the best treatment path. For any bariatric surgeries taking place on or after January 1st, 2018, you need to complete a second opinion with Included Health prior to scheduling surgery and initiating precertification in order for it to be covered under the Bread Financial medical plan. If you don’t complete the second opinion prior to your surgery, you will be responsible for 100% of your bariatric surgery expenses. It is recommended that you begin a second opinion with Included Health at the beginning of the multi-month precertification process, before surgery. The process takes about 3 weeks.

The procedure must also be completed at an Aetna IOQ in order for the cost to be covered by the Bread Financial Medical Plan.

All Included Health services can be easily accessed by calling 1-855-229-0828, visiting includedhealth.com or by downloading the Included Health mobile app. For certain Included Health services, a member from your Care Team will reach out to you to learn about your exact needs. They’ll take care of everything, from finding you the right doctor to collecting your medical records. Expect a care coordinator to follow up with you after you’ve seen the recommended doctor, to answer any questions you may have and start additional cases if needed.

Included Health physicians span all conditions and specialty areas, including: back and knee pain, cancer, heart disease, arthritis, behavioral health disorders, migraines, digestive system disorders, dermatology, pediatrics, ACL tears, and much more.

Included Health selects physicians based on their education and training, patient outcomes, and location. As a result, we’re able to match you with highly experienced doctors and specialists who are best suited to your personal preferences and medical needs.

Typically, you’ll only pay your regular copay and office visit fees for seeing an in-network doctor. For further details, call the Included Health Care Team at 1-855-229-0828, available Monday through Friday from 8:00am – 9:00pm ET.

Most doctors are glad to get the opinions of their peers when assessing complicated medical issues. Their primary goal is to get you feeling better. A good way to frame a conversation with your treating physician regarding a Included Health expert opinion is:

  • “I requested a consultation about [condition] and here’s what I learned…”
  • “Here’s what I was surprised to find out…”
  • “Dr. Smith recommended these options for my care because of these reasons…”

Any personal health information you share with Included Health is held confidentially and will not be shared by us, unless authorized by you, or where legally permissible and/or required under applicable law.
