Common questions about our health benefit

  • You’ve received a diagnosis or have been recommended for surgery or a course of treatment. We can provide you and your local treating physician an expert second opinion from a leading expert specializing in your area of need.
  • You need a primary care physician or specialist for an in-person Office Visit. We’ll get you into the office of one of the leading doctors within your benefit plan’s network and even set the appointment for you.
  • You or a covered dependent is in the hospital and would like clarity on the care you are about to receive. We will reach out directly to you, your family and your local care team to provide prompt support.

Included Health is available to AT&T employees, non-Medicare eligible former employees, select Medicare eligible retirees and eligible dependents who are enrolled in an AT&T sponsored medical plan. This includes self-insured plans such as the AT&T Medical Program—Bronze, Silver, or Gold Option. Some services may be limited if you are not enrolled in an AT&T sponsored self-insured medical plan.

Included Health is provided by your Plan. Its services are available at no additional cost to you and your covered dependents. Regular cost sharing provisions (for example coinsurance) will apply for any treatments or procedures performed as a result of the Expert Opinion or for an in-person Office Visit arranged through Included Health – Included Health will find the doctor, collect all relevant medical records, and (if necessary) schedule the appointment free of cost.

When you reach out to Included Health to get started, you will be assigned a Care Team which is comprised of a Staff Physician, Care Coordinator and Records Specialist. Your Care Team will take care of all the logistics of your case, from finding the right expert physician to collecting your medical records. After your local treating physician has received the Expert Opinion from the selected expert physician (with a courtesy copy to you), your Care Team will follow up with you to answer any questions and make sure your needs have been met, and start any additional cases if necessary.

Included Health expert physicians span many conditions and specialty areas, including, for example: neck and back pain, knee-related issues, cancer, heart disease, arthritis, behavioral health disorders, chronic headaches or migraines, digestive system disorders, dermatology, and pediatrics.

Included Health expert physicians are selected on a number of different criteria, such as their institution, training, patient outcomes and procedure volumes. As a result, Included Health connects you with only top-ranked doctors that specialize in your exact area of need.

Expert Opinions are generally completed within 2-3 weeks.

Most treating physicians are highly receptive to receiving another opinion on a complicated medical issue. You should not be concerned about offending your local physician as their primary goal is to help you get better, and this opinion is simply an additional resource to do so. A good way to frame the conversation with your local treating physician regarding the Expert Opinion is:

  • “I requested a second opinion about [condition] and I’d like to discuss the information with you.…”
  • “Here’s what I was surprised to find out…”
  • “Dr. Smith recommended we consider these options for my care because of these reasons…”

Yes, as long as they are an eligible covered dependent enrolled in a sponsored AT&T medical plan, you can initiate a case for them. Or if they are over the age of 13, they can register for an account and initiate a case themselves online, through the mobile app or over the phone.

When contacting Included Health for a certain service, you have the ability to confirm providers that you have previously seen regarding your condition or the area of interest. Your Care Team will request for your consent to begin contacting these providers to obtain the relevant medical records, which will be used by your Care Team and Included Health expert physicians to review your case.

No, Included Health does not provide information on the medical cost for any recommended treatment options. Cost information will need to be provided by your actual local treating physician.

You have no obligation to follow the recommendations provided by Included Health. This is simply another resource available to you and your local treating physician to obtain additional information and discuss your treatment options together.

Any personal health information you share with Included Health is held confidentially and will not be shared by us, unless authorized by you, or where legally permissible and/or required under applicable law.

The last four digits of your SSN are needed to confirm both your identity when creating an

account and your eligibility for the Included Health benefit.
